Description
create an Uploader the same as all other Dijits are created.

(In [17482]) Fixes #8784, #8790, #8791, #8908, #9238, #9239, #9240, #9241, #8375, #8390 - FileUploader? major upgrade to 1.4. Primary upgrade is that the uploader is now a widget and the button is recreated in Flash to avoid the wmode bugs and css position jankiness. \!strict
